Merit Health Wesley: Able to serve more veterans

Hattiesburg - Merit Health Wesley was recently awarded the Veterans Health Administration (VHA) Community Based Outpatient Clinic under the Patient Aligned Care Team (PACT) model.

PACT is the cornerstone of the New Models of Care transformation initiative intended to transform the way Veterans receive their care.  It assists VHA in transforming Veterans' care by providing patient-driven, proactive, personalized, team-based care oriented toward wellness and disease prevention resulting in improvements in Veteran satisfaction, improved healthcare outcomes and costs.  The PACT model is built on the well-known concept of the patient centered medical home staffed by high-functioning teams.

A Patient Aligned Care Team is each Veteran working together with health care professionals to plan for life-long health and wellness.
 
They focus on:
• Partnerships with Veterans
• Access to care using diverse methods
• Coordinated care among team members
• Team-based care with Veterans is the center of their PACT

A PACT is a partnership between Veterans and their health care teams to make sure Veterans receive whole-person care to meet a patient's individual health care goals. The emphasis is on prevention and health promotion. Using a team-based approach, PACT puts the patient at the center of the care team which also includes the patient's family members, caregivers and health care professionals—primary care provider, nurse care manager, clinical associate, and administrative clerk.
